If you are a Treatment Facility, your listing level would depend on your marketing objectives and budget and the services and amenities your facility provides. I am therapist, but work in an office with 3 others. How should I create my listing? Under my name, or under the name of the practice?

You can create your listing either way. If you want to promote yourself, you should make the listing in your name, particularly if you may be changing jobs sometime in the future. The practice can be listed on your page or you can list the practice and have your name and include the names of your co-workers on the page. You can edit the page at your discretion should someone leave or you expand your practice.
