Program Narrative
Project Transition is a community-based residential treatment program with locations throughout Pennsylvania. We provide rehabilitation for adults who struggle with serious mental illness and co- occurring substance abuse issues. Our programs foster sustainable recovery through a comprehensive approach promoting restoration of independence and reintegration into the wider community.
